NALA Provider Profile
This survey is the only attempt in Nebraska to gather information about the services that you provide. To be representative of the profession in Nebraska, it is important that a large percent of membership complete the survey. Please take a few minutes and participate in the Provider Profile Project – a project of the Nebraska Assisted Living Association.
Survey of Membership Service Needs
Save the document to your computer to fill out and then e-mail back to NALA as an attachment or print off the document to fill out and fax or mail back to NALA. The survey will also be distributed during the May NALA District Meetings and can be filled out at that time. The information tabulated from this survey will be used by NALA committees and staff to plan future Association activities. Return by June 9, 2008.
