Nurse Aide Testing Procedures

For LICENSE RENEWALS:
You may self register here. You must provide verification of a lapsed status on the Nebraska Nurse Aide Registry with registration or prior to testing.
Instructions for obtaining verification:

  1. Go to the Nebraska Department of Health and Human Services License Information System Search http://dhhs.ne.gov/publichealth/Pages/lis_lisindex.aspx.
  2. For search “Type”, click “Individual”
  3. For “License Status”, click “All”
  4. Perform a search for your individual license information.
  5. Click on your name to view the license details and other information.
  6. Click the link titled “Generate a verification of this license.”
  7. Mail or Fax your verification to NHC-LC, or when the printable verification page displays, copy the URL from the address bar of your browser and paste it into an email to NHC-LC.

For FACILITY COURSE NOTIFICATIONS:
If you have provided training at an approved facility, you can submit your students for testing using this form. This verifies their eligibility to test, and they will then need to self-register for testing here. Download a registration form for mailing or faxing here.

For OUT OF STATE REGISTRANTS:
People who have taken the NA class from outside of Nebraska must contact the NA registry to establish eligibility: http://dhhs.ne.gov/publichealth/Pages/crl_nursing_na_na.aspx.
