Revised 2018. The Assisted Living Resident Rights booklet describes the rights of the resident and responsibilities related to these rights. It provides the resident and/or their designated representative written information about rights that residents in an assisted living facility are guaranteed under state law.
The booklet includes a tear-out advance directives form and resident rights acknowledgment form for the resident’s signature. It also includes the new Notice of Nondiscrimination and Language Assistance Taglines that are required as part of the Nondiscrimination in Health Programs and Activities Final Rule. It is sold in packages of 25 and available in standard-print.
